Birth/Death Certificate Application
The Montana Office of Vital Records issues birth and death certificates, makes corrections to birth certificates, processes certificate of adoption, provides search services to help determine the county a marriage license or divorce decree was issued in.

You may order a copy of a Montana Birth/Death Certificate using one of the following services listed below:
