Long Term Care Facilities

Skilled Nursing Facilities (SNFs) and Nursing Facilities (NFs) are required to be in compliance with the requirements at 42 CFR Part 483, Subpart B, to receive payment under the Medicare and Medicaid Programs. Currently, Montana utilizes the Standard Survey process for the completion of the surveys. The Standard Survey process consists of three components of investigation, which include: observation, interview and record review. These components provide the evidence of the facility's compliance or non-compliance.

Montana Reportable Incidents Portal  Effective 02/10/2022  -  If you are having issues logging into the reporting portal, enter your username (email address) and click on "Forgot Password".  A temporary password will be emailed to you. The reporting portal functionality is best supported by Google Chrome, Microsoft Edge or Mozilla Firefox. Due to system maintenance, historical reporting data is currently unavailable and you may not be able to enter your 5 day findings for previous reports until 2/14/2022.
