Plan Review Forms

Completion of a Plan Review Form is necessary for review by EHFS:    

1. Complete all sections of the form
2. Gather all required documents and attachments (described on Page 2 of form)
3. Print and then fax or mail the Plan Review Form and all required documents to EHFS and your local environmental health department
4. OR email the Plan Review Form and all required documents to EHFS and your local environmental health department

• Temporary/Special Event Campground Plan Review Form - Temporary campground plans are reviewed and permitted by local environmental health offices, not EHFS. Please inquire with them prior to completing this form to ensure you are using the correct version for your jurisdiction.
