Delayed Birth Certificates
How to Apply
To begin the process, you will need to submit the following to our office:- The Application: Complete the Delayed Birth Certificate Form.
- Supporting Evidence: Please include any original documents (such as baptismal records, hospital records, or school records) that serve as evidence of the birth.
- Note: All original documents will be handled with care and returned to you via mail once the process is complete.
- Registration Fee: A fee of $41.00 is required. This fee includes one certified copy of the new birth certificate once it has been processed and filed.
