Team Care

Team Care is a program for Montana Medicaid and HMK Plus members where they are assigned a medical team (one primary care provider and one pharmacy) that they are required to use for all their prescriptions.

The Team Care program encourages and supports Medicaid and HMK Plus members and providers to establish a strong doctor/patient relationship and ensure the appropriate use of Medicaid and HMK Plus service.
