Complaint Procedures
If you have a complaint about a facility that is licensed and/or certified by the Montana Department of Public Health and Human Services, please submit the appropriate form below to the Quality Assurance Division, PO Box 202953, Helena MT 59620-2953.
If you have questions about the complaint process, please call the Certification Bureau at 406-444-2099 or the Licensure Bureau at 406-444-2676.
Be sure to use the correct form to ensure that your complaint remains confidential and is addressed promptly.
