Thank you for visiting our website to find out how to receive a copy of your official immunization record. Montana has an Immunization Information System (IIS) called imMTrax that houses all children's/adults immunization records that have been consented into the registry.
To locate your current immunization record please contact your local health department or clinc that you received immunization at by clicking the link below. If the local health department is unable to locate your record, please ask to have your record input into the IIS so your record will be kept for ever.
