To obtain your immunization records please contact your local health department or the clinic that administered your immunizations by clicking the link below. By inputting your records in imMTrax participating providers will always be able to access your immunization information. imMTrax ensures that immunization records are complete and accessible, even if patients relocate, change providers, or misplace their personal "shot cards."
