How To Make a Referral

Montana Chemical Dependency Center
is the single state chemical dependency treatment facility administered by the Department of Public Health & Human Services, Addictive & Mental Disorders Division. It is a 50 bed in-patient treatment facility operating 24 hours per day, 365 days per year.

  • Only Montana State Licensed Addiction Counselors may submit referrals to MCDC
  • A referral packet must include a current biopsychosocial assessment, Level of Care Index which meets the American Society of Addictive Medicine (ASAM) Level III.5 and III.7 Dimensional Analysis, a Biopsychosocial assessment, and drug use history.
  • Referral packets are submitted to MCDC Admission's Coordinator.
  • Complete referral packets, which meet appropriate placement criteria, will be accepted for treatment at MCDC and scheduled for admission.

Generally, all admissions are assigned arrival times between Sunday through Thursday 8:00 a.m. to 4:00 p.m. Unique circumstances warranting alternative admission times may be requested and will be accommodated as much as possible.
