For those of you using the paper abstracts;
On page @2: "Post ED destination:
If it's still there, on the line Admitted to ; please delete the "23 hour" choice.
It's confusing and needs to be eliminated as a choice. Patient with injuries who are admitted to your facility must either be admitted to ICU, be between the ages of 0-4years, or be hospitalized > 48 hours to meet inclusion criteria. The 23-hour admission doesn't meet inclusion criteria in and of itself, so let's GET RID OF IT.
I have deleted the "23 hour" on the paper forms I send out to facilities just starting to use the abstracts, but some of you have the older version with that choice still on it.

Source of Trauma System Inclusion Designation = Source of Trauma Team Activation or not
1 = Patients for who Trauma Team Activation occurred as a result of Prehospital notice from the field. (These are NOT patients who arrived by Private Vehicle or Walk-in)
2 = Patients for whom Trauma Team Activation occurred upon arrival @ emergency department , irrespective of how they arrived @ ED. (If the patient was transported by any form of EMS, but there was no prior notice to activate the Trauma Team before the patient arrived, & then a Trauma Team Activation occurred when in ED, that patient is a "2")
3 = Patients for who Trauma Team Activation occurred because the patient was a transfer
4 = Patients for who Trauma Team Activation occurred when the patient was transferred to Trauma Service from another service within the hospital
5 = Patients who met other Trauma Registry Inclusion criteria, but never had a Trauma Team Activation
