Extended Facility Information
The information on this page is to be used by Facilities that currently have an extended registration/license.
- Facilities that wish to apply for an extended registration/license need to contact their Child Care Licensor in writing.
Extended Facilities must provide the following annually:
Extended Facilities must provide New Employee Information within 15 days of hire.
