Change of Status Application

The following forms/information are included in a Change of Registration Application for Family, Group, and Center Facilities:

  • Change of Registration/License Checklist (Print First)
  • Change of Status Application
  • W-9
    • W-9 Instructions
  • Insurance Verification Form
  • Floor Plan / Square Footage Report

All Change of Status Application Packets should be submitted to the Child Care Licensor for your County:
