Case Management Services

Who?

Ensuring that the services people receive meet their unique needs and that they are delivered in an efficient, non-duplicated and coordinated manner is a critical component of DDP-funded services. DD Case Management helps ensure quality services by assessing the needs of persons with developmental disabilities, helping people access needed services and monitoring the services received to ensure that the person's needs are met. Each month, the DDP provides or contracts for case management services statewide to approximately 2,230 persons with developmental disabilities who are 16 years of age and older, including those people who are waiting for DDP-funded services. DD Case Management is Medicaid-funded, and is an entitled service for those persons meeting state-established criteria.

What?

The purpose of Case Management is to assess, coordinate and assure the delivery of services and supports required by individuals with developmental disabilities who are 16 years of age or older. The individual, in partnership with the case manager, service staff and significant others, establishes priorities and participates in the development, implementation and evaluation of and Individual Plan (IP). The IP is the foundation for a dynamic, evolving process that guides services and establishes parameters of accountability. Case Management is intended to assist individuals to gain access to needed medical, social, educational and other services; enhance independence and interdependence with others in the community; expand options for participation in and contribution to society; increase skills and abilities to enable on-going personal growth; and assist individuals to reside, work and play in safe, healthy, integrated environments.

You may also obtain additional information about the location of contracted Case Management Services by contacting the service provider agencies listed below. Once eligibility for services has been established, a developmental disabilities case manager will assist you in the process of entering other DDP services.
