This Medicaid Home and Community-Based Waiver service provides support coordination and an enriched menu of support services for families of children (up to age 22) who have very severe disabilities. The primary goal of the program is to enable families to keep their child or young adult in the family home. Services available under this program include things such as home modifications, habilitation and expanded respite care services. As of July 1998, 154 children received this service.
Intensive Family Education and Support services makes available enhanced levels of support coordination and services to families of children with severe disabilities. The individuals served must be determined to require intensive levels of support in order to remain at home and avoid placement in an institution (ICF/MR) in order to be eligible for the program.

Once eligibility for services has been established and a service opportunity becomes available, a Family Support Specialist from the Family Education and Support agency will assist you in the process of entering services.
