How to Add Services to a Current DDP Provider Contract
For a qualified provider of DDP services with current contract to be authorized to provide additional services, you must determine which additional services you are able to provide under a particular funding source. See the links under How to become a new DDP Provider for specific waiver(s) and applicable Department rules.
Once you determine which additional services you can offer while meeting Department rules, you must fill out an application to add services to a current DDP Provider contract.
OR For existing DDP qualified providers to add licensed/certified services to a current contract with the Developmental Disabilities Program (DDP), you must use the provider application to add licensed/certified services to a DDP Provider contract.
