Targeted Case Management

The Consolidated Omnibus Budget Reconciliation Act of 1985 (Public Law 99-272) authorizes case management services for six special categories of recipients of Montana Medicaid. The categories include High Risk Pregnant Women recipeints of Public Health Home Visiting, (PHHV) and Children with Special Health Care Needs.

Sections I, II and VI from the Targeted Case Management Provider Manual are the references that introduce targeted case management, and define high-risk pregnant women and children with special health needs.

In order for a public health department to bill Medicaid for targeted case management services, the department needs to do the following things:

1. Apply for a Medicaid provider number for targeted case management; please see the medicaid web portal link:

2.  The targeted case manager is responsible for using the most current diagnosis book and for billing the appropriate codes for more information see the targeted case managment-non mental health link:
